Information on the factors influencing parents’ decision-making process following a lethal, life-limiting or severely debilitating prenatal diagnosis remains deficient. A comprehensive systematic review and meta-synthesis was conducted to explore the influencing factors for parents considering termination or continuation of pregnancy following identification of lethal, life-limiting or severely debilitating fetal abnormalities. Electronic searches of 13 databases were conducted. These searches were supplemented by hand-searching Google Scholar and bibliographies and citation tracing. Thomas and Harden’s thematic synthesis method was used to synthesise data (...) from identified studies. Twenty-four papers were identified and reviewed, but two papers were removed following quality assessment. Three main themes were identified through systematic synthesis. Theme 1, entitled ‘all life is precious’, described parents’ perception of the importance of the fetus’ life, a fatalistic view of their situation alongside moral implications as well as the implications decisions would have on their own life, in consideration of previous life experiences. Theme 2 contained two sub-themes which considered the parent’s own imagined future and the influence of other people’s experiences. Finally, Theme 3 presented three sub-themes which may influence their parental decision-making: These described parental consideration of the quality of life for their unborn child, the possibility of waiting to try for another pregnancy, and their own responsibilities and commitments. The first review to fully explore parental decision-making process following lethal, life-limiting, or severely debilitating prenatal diagnosis provided novel findings and insight into which factors influenced parents’ decision-making process. This comprehensive and systematic review provides greater understanding of the factors influential on decision-making, such as hope, morality and potential implications on their own and other’s quality of life, will enable professionals to facilitate supported decision-making, including greater knowledge of the variables likely to influence parental choices. One of the gaps in the current international marketing literature is in the area of consumer ethics. Using a sample drawn from Japanese consumers, this study investigates these individuals' reported ethical ideology and their perception of a number of different ethical situations in the realm of consumer behavior. Comparisons are then made across several demographic characteristics. The results reveal differences which provide theoretical support for expanded research in the area of cross-cultural/cross-national consumer ethics and highlight the need for managers to (...) consider possible differences in the ethical behavior of consumers when entering a new international market. Using a longitudinal case study design, we have tracked the recovery of motor function following severe traumatic brain injury (TBI) through a multimodal neuroimaging approach. In 2006, Canadian Soldier Captain (retired) Trevor Greene (TG) was attacked with an axe to the head while on tour in Afghanistan. TG continues intensive daily rehabilitation, which recently included the integration of physical therapy (PT) with neuromodulation using translingual neurostimulation (TLNS) to facilitate neuroplasticity. Recent findings with PT+TLNS demonstrated that recovery of motor function occurred (...) beyond conventional time limits, currently extending past 14-years post-injury. To investigate whether PT+TLNS similarly resulted in associated cognitive function improvements, we examined event-related potentials (ERPs) with the brain vital signs framework. In parallel with motor function improvements, brain vital signs detected significant increases in basic attention (as measured by P300 response amplitude) and cognitive processing (as measured by contextual N400 response amplitude). These objective cognitive improvements corresponded with TG’s self-reported improvements, including a noteworthy and consistent reduction in ongoing symptoms of post-traumatic stress disorder (PTSD). The findings provide valuable insight into the potential importance of non-invasive neuromodulation in cognitive rehabilitation, in addition to initial indications for physical rehabilitation. Nursing Inquiry 2012; 19: 63–70 Football reminiscence for men with dementia: lessons from a realistic evaluationA major challenge of studies of non pharmacological dementia interventions is the likely modest intervention effect size and difficulties collecting data from individuals with behavioural, psychological and communicative symptoms. The reported Realist Evaluation is built around sets of contextually comparable case studies of Football Focussed Reminiscence for men with dementia. The study aim was to evaluate benefits of football related reminiscence (...) for individuals and family carers. Four case studies were constructed; two community groups, one nursing home groups and individual sessions within their family home. Data was collected as field notes from non‐participation observation of reminiscence sessions, notes of conversations with people with dementia, audio recorded interviews with family members, facilitators and dementia link workers. The analysis was framed around identification and extraction of data pertaining to Context‐Mechanism‐Outcome configurations within each data set, and patterns and threads were compared across the cases. An important finding was that anticipation of pleasure in tandem with a sense of continuity appeared to be important mechanisms triggering optimal benefit. The paper explores design considerations, project delivery experiences and the contribution that innovative theory driven research adds to the study of complex interventions including those with behavioural and communicative symptoms of dementia. There is little (...) systematic study of nurses’ day‐to‐day practice of nursing patients with delirium. The aim was to uncover the kinds of knowledge that informs nurses’ care and to explicate the basis of that knowledge. Critical Discourse Analysis is underpinned by the premise that powerful interests within society mediate how social practices are constructed. Links were made between the grammatical and lexical features of nurses’ language about care in interviews and naturalistic settings, and the healthcare context. Care focused on the continuous surveillance of patients with delirium by nurses themselves or vicariously through other patients, and containment. Nurses influenced by major discourses of risk reduction and safety, constructed patients with delirium as risk objects. The philosophy of person‐centred and dignified care advocated in nursing literature and government policy is an emerging discourse, though little evident in the data. The current dominant discourses on safety must give space to discourses of dignity and compassion. (shrink)

Contexte : L’autonomie est un concept central tant en bioéthique qu’en réadaptation. La bioéthique a mis l’accent sur l’autonomie en tant qu’auto-gouvernance et sur son application dans la prise de décision en matière de traitement. En plus de discuter de l’autonomie décisionnelle, la réadaptation met aussi l’accent sur l’autonomie en tant qu’indépendance fonctionnelle. Dans la pratique, la prise en charge des patients en perte d’autonomie est une composante importante des soins de réadaptation, mais elle engendre aussi des tensions et des (...) défis. Notre objectif était de mieux comprendre les façons complexes et distinctes dont l’autonomie est comprise et maintenue dans le contexte des soins de réadaptation en examinant comment l’autonomie est abordée dans la littérature sur la réadaptation. Méthodes : Nous avons effectué une étude de la portée des questions d’autonomie dans le contexte de la réadaptation mentale et physique. Notre processus s’est déroulé en trois étapes séquentielles. Nous avons extrait et analysé des informations bibliométriques. Nous avons ensuite examiné comment l’autonomie était définie et conceptualisée. Enfin, nous avons examiné la façon dont les articles traitaient du rôle des professionnels de la santé en réadaptation dans l’autonomie des patients. Résultats : Les articles comprennent 16 rapports empiriques, 17 études de cas et 30 articles théoriques. Les récits conceptuels les plus courants sur l’autonomie s’appuyaient sur le principisme, les analyses fondées sur les droits et les analyses juridiques et les approches relationnelles/sociales. Nous avons identifié quatre grandes approches pour répondre à l’autonomie du patient : le soutien, la promotion, le respect et la promotion. Conclusion : Cet examen aide à clarifier certaines des ambigüités et des distinctions conceptuelles qui sous-tendent les discussions et les pratiques liées à l’autonomie en réadaptation. Il attire également l’attention sur un large éventail d’activités que les professionnels de la santé peuvent entreprendre dans le but de soutenir, de promouvoir, de respecter et de promouvoir l’autonomie des patients dans les soins de réadaptation. Dans le contexte actuel de longs délais d’attente pour accéder aux services de réadaptation, la grande majorité des établissements utilisent la priorisation des demandes de référence pour aider à gérer les listes d’attente. Les pratiques de priorisation varient grandement d’un milieu à l’autre et il y a peu de consensus sur la meilleure façon de prioriser les demandes de référence. Cet article décrit les processus de priorisation de services en physiothérapie au Québec et leurs implications potentielles en termes d’équité dans (...) l’accès aux services. Il s’agit d’une analyse secondaire d’une enquête menée en 2015 auprès des cliniques externes en physiothérapie (n=98 ; la proportion de participation était de 99 %) dans les hôpitaux publics du Québec. Dans de nombreux milieux, les personnes ayant une condition orthopédique aigüe étaient priorisées, tandis que les conditions chroniques recevaient une priorité moindre. Nous avons comptabilisé 72 combinaisons différentes de critères de priorisation utilisées dans les cliniques externes de physiothérapie. Nous avons également observé une variabilité dans le type de personnel impliqué dans le processus de priorisation, le nombre de niveaux de priorité utilisés pour classer les demandes de référence et la source d’information utilisée pour établir la priorisation. Ces résultats mettent en lumière des enjeux potentiels concernant l’équité dans l’accès aux services de physiothérapie : la priorisation des personnes atteintes de conditions aigües au détriment de celles atteintes de conditions chroniques, l’absence de consensus sur un processus de priorisation équitable et la nécessité d’évaluer adéquatement les besoins de traitement des patients. De futures recherches et interventions sur les critères et les processus de priorisation sont nécessaires pour assurer un accès équitable aux services de physiothérapie, en particulier dans le secteur public. In the UK and elsewhere suicide presents a major cause of death. In 2008 in the UK the topic of suicide rarely left the news. Controversy surrounding Daniel James and Debbie Purdy ensured that the problem of assisted suicide received frequent media discussion. This was fuelled also by reports of a higher than usual number of suicides by young people in South Wales. Attention attracted by cases such as that of Daniel James and Debbie Purdy can lead to (...) a neglect of the problem of how to respond to the vast majority of suicides, in which there is no obvious accompanying health problem. This paper seeks to redress that balance to some degree. Its focus is primarily on the provision of assisted suicide when the person seeking such assistance has no accompanying health problem. It is argued that a network of suicide centres (SCs) should be established, the purpose which in certain circumstances, will extend to assisting suicide in cases where a competent person has a definite, sustained intention to end their own life. Arguments in support of this proposal are provided, focusing on respect for autonomy, relief of suffering and respect for the ‘life-plan’ of a person. Considerations relating to reduction of harms to third parties are also recruited. Eight responses to the proposal are considered. But none is found compelling, nor are they found jointly compelling. In locating friendly fire in contemporary philosophy of religion, D.Z. Phillips shows that more harm can be done to religion by its philosophical defenders than by its philosophical despisers. Friendly fire is the result of an uncritical acceptance of empiricism, and Phillips argues that we need to examine critically the claims that individual consciousness is the necessary starting point from which we have to argue: for the existence of an external world and the reality of God; that God is a (...) person without a body, a pure consciousness; and that to assent to a religious belief is essentially to assign a truth value to a proposition independent of any confessional context. When these products of friendly fire are avoided, we arrive at a new understanding of belief, trust and the soul, and refuse to say more or less than we know about the realities of human life in the service of religious apologetics. Eugenics casts a long shadow over contemporary genetics. Any measure, whether in clinical genetics or biotechnology, which is suspected of eugenic intent is likely to be opposed on that ground. Yet there is little consensus on what this word signifies, and often only a remote connection to the very complex set of social movements which took that name. After a brief historical summary of eugenics, this essay attempts to locate any wrongs inherent in eugenic doctrines. Four candidates are examined and (...) rejected. A common intuition about evidence is that if data x have been used to construct a hypothesis H, then x should not be used again in support of H. It is no surprise that x fits H, if H was deliberately constructed to accord with x. The question of when and why we should avoid such “double-counting” continues to be debated in philosophy and statistics. It arises as a prohibition against data mining, hunting for significance, tuning on the signal, and (...) ad hoc hypotheses, and as a preference for predesignated hypotheses and “surprising” predictions. I have argued that it is the severity or probativeness of the test—or lack of it—that should determine whether a double-use of data is admissible. I examine a number of surprising ambiguities and unexpected facts that continue to bedevil this debate. (shrink)

This chapter deals with the ways in which entrepreneurships driven by industrial design and the strategies for interconnecting design for innovation are articulated. In the first instance, current practices are recognized in order to analyze the different processes that entrepreneurial dynamics go through. This analysis is carried out from a systemic perspective, at three levels of abstraction: designer, entrepreneurship and ecosystem. The theoretical and conceptual framework will be structured based on the innovations driven by design, which occur around the designer, (...) his personal projects, and the possible interactions and singularities offered by living spaces. The objective of this work is to unravel the eventual relationships of the entrepreneurial designer in the ecosystem and to highlight the potential added by emerging connections, which allow for the projection of synergistic actions with other actors to create value(s). In this way, other possible views of the entrepreneurial process are provided, where the cultural and philosophical dimension that characterizes industrial designers allows them to achieve a higher quality of life, designing and producing collectively (in) their environment. We report the results of a randomized trial to assess the impact of an innovative ethics curriculum on the knowledge and confidence of 85 medical house officers in a university hospital programme, as well as their responses to a simulated clinical case. Twenty-five per cent of the house officers received a lecture series, 25 per cent received lectures and case conferences, with an ethicist in attendance, and 50 per cent served as controls. A post-intervention questionnaire was administered. Knowledge scores did (...) not differ among the groups. Confidence regarding ethical issues was significantly greater in the aggregate intervention group compared to the control group. Confidence regarding procedural issues related to ethics was significantly higher for the EI group than for the controls. Responses to a simulated case showed that significantly fewer house officers in the EI group would intubate a patient for whom such therapy would be futile. We conclude that ethics education can have an impact on house officers' confidence and their responses to a simulated case, and that the EI was more effective than the LI. Such results have implications regarding the implementation of ethics education during residency. (shrink)

I discuss a tension between two broadly Aristotelian ideas about the role of emotions in virtue and consider its implications for the original and attractive theory of virtuous judgment that Gopal Sreenivasan develops in Emotion and Virtue. One is the idea that a virtuous person has fitting emotions. The other idea is that the virtuous person has emotions that point her toward performing a virtuous action. I explain the tension between these ideas, and how it arises with respect to both (...) of Sreenivasan's central examples of virtue: compassion and courage.I suggest that this tension generates some interesting and systemic respects in which a virtuous agent's virtuous emotional responses hamper her attempts to judge what is the virtuous thing to do. This makes me less sanguine than I take Sreenivasan to be about the contributions of emotion to the virtuous agent's reliability in passing his “Central Test of Virtue.”. (shrink)

In practice, the inquiry into whether Socrates/William Tell/Robin Hood really existed hinges on the origin of the causal sequence leading up to the present use of the name employed for the personage in question, even though the occurrence of a Kripkean ‘baptism’ is not a logical consequence of most assertions employing that name, but only an ‘implication’ like the sort that makes examples of Moore's paradox (e.g. 'It is raining, but I have no idea whether it is raining’) strike ordinary (...) language users as ‘inconsistent’. This paper, written from the perspective of a liberal Protestant, addresses Karl Barth’s famous dispute with his predecessor Friedrich Schleiermacher. It claims that despite the vehemence he often employs against Schleiermacher, a careful assessment of Barth reveals an ever-shifting ambivalence. Barth’s dismissal was very much a heated reaction amidst post-war circumstances, and Schleiermacher served as a symbol for aspects of pre-war culture and theology that are not necessarily reflective of Schleiermacher’s original project. Funding organizations around the world are adopting open science policies, resulting in a pressing need for open science programs. In response to the 2011 decadal survey, NASA sought to expand and accelerate omics research, releasing its GeneLab Strategic Plan in 2014. GeneLab is an open science data repository and analysis portal for spaceflight and space-relevant omics data. GeneLab’s output has been outstanding, but its full potential as a way to transform space biology has not yet been achieved. End-of-Life Care and Pragmatic Decision Making provides a pragmatic philosophical framework based on a radically empirical attitude toward life and death. D. Walter Wink is well-known for his exposition of what he has called “the third way” in Christian ethics, and his three-volume theological work expounding the nature of “the dominating powers that be” has left a lasting mark on Christian (post)liberalism -- not just in North America but elsewhere as well. However there is a vexing philosophical tension in Wink’s work, one that is even reflected in the fading of mainline Protestantism as a social force. Zusammenfassung In diesem Artikel wird das neoliberale Modell von Rationalität dem Verständnis der Rationalität gegenübergestellt, das Fichte in der Grundlage der gesamten Wissenschaftslehre vorschlägt. Der erste Teil dreht sich um Foucaults Verbindung zwischen Gouvernementalität und Rational Choice Model. Im zweiten Teil werde ich die Handlungstheorie von Ludwig von Mises erörtern, die er in seinem Meisterwerk von 1940, Nationalökonomie. Eine Theorie des Handelns, aufarbeitet. Denn obwohl er die Rational Choice Theory nicht selbst formulierte, gilt er als beispielhafter Vertreter eines anthropologischen Musters, (...) in dem menschliches Handeln, Wahl, Rationalität und Wirtschaft eng voneinander abhängen. Im dritten Teil des Artikels kommt J. G. Fichte ins Spiel und seine in der Grundlage vorgeschlagene Rationalitätskonzeption zur Geltung. Denn die ganze Anthropologie von von Mises, auf welche sich die Möglichkeit des vernünftigen Handelns des homo oeconomicus überhaupt stützt, kann darauf zurückgeführt werden, was Fichte allein im zweiten, theoretischen Teil der Grundlage darstellt, dem Fichte allerdings einen dritten, praktischen Teil folgen lässt, in dem er die Gründung und die Rechtfertigung, ja die authentischen Bedingungen der Möglichkeit jenes theoretischen Handelns bzw. der Intelligenz in der Entfaltung der praktischen Vernunft darstellt. Critical factors that appear to encourage vocal development in humans are altriciality and long immaturity. Hominid infants appear to have evolved a specific tendency to use elaborate vocalization as a means of soliciting long-term investment from caregivers. The development of such vocal capacity provides necessary infrastructure for language development across human life history.

The central thesis of JC Beall’s paraconsistent Christology is that Christ, being human and divine, is a contradictory being, and a rational Christology can accept it, since logic nowadays does not exclude the possibility of true contradictions. In this paper, I move from Beall’s theory and I present an alternative view. I quote seven statements of the so-called ‘Athanasian Creed’ which synthesizes the results of conciliar Christology. The aim of the Creed is to combat monophysitism by stressing the duplicity and (...) unity of Christ: two (incompatible) natures inseparably joined in only one person. I note that the two-in-one principle, so intended, may be seen as an ancestor of what has been called ‘conjunctive paraconsistency’, whereby there could be true contradictions but contradictories cannot be separately true. I specifically oppose this view to Beall’s idea of Christ’s human divinity (or divine humanity) as a glut, showing that in the conjunctive account, true contradictions do not require any overlapping or joint ascription of truth and falsity. This article critically examines Foucault's engagement with neoliberalism. While Foucault declares that his analysis of this tradition is primarily descriptive, I argue that he continually questions whether neoliberalism is less disciplinary and biopolitically normalizing than traditional forms of liberalism. Although Foucault does not endorse neoliberalism as a prescriptive solution to these problems of normalization, his interest in such problems is consistent with his tendency to privilege freedom over other values like justice and equality. This helps to clarify the normative stakes (...) of Foucault's analysis while rejecting any suggestion that he was invested in neoliberalism as a comprehensive political program. Indeed, I repudiate the claim that he was “seduced” by neoliberalism. Furthermore, I reject the idea that he was trying to “invent” a distinctively socialist governmentality through the prism of neoliberalism. Finally, I consider the broader significance of this discussion for normative political philosophy. (shrink)

In Reconstruction in Philosophy, John Dewey issued an eloquent call for contemporary philosophy to become more relevant to the pressing problems facing society. Historically, the philosophy of a period had been appropriate to social conditions, but despite the vast changes in the contemporary world and the complex challenges confronting it philosophy had remained ossified. Karl Popper also was dissatisfied with contemporary philosophy, which he regarded as too often focusing upon “minute” problems. Both Dewey and Popper, however, were optimistic that the (...) situation could be turned around. In this essay D.C. Phillips argues that the resources they mustered give no basis for this optimism; in particular, Phillips emphasizes that philosophy cannot have traction with closed‐minded or fanatical individuals. Dewey passed over cases where his ideas about democratic processes and free intellectual exchange faced intractable difficulties, according to Phillips, and he further suggests that Popper “waffled” over the so‐called “myth of the framework.”.
